I, for good value and consideration, hereby give permission to Get Pultz'D, LLC, to make audio video, and still recordings of myself (or my minor child), as the case may be, for the purposes of internal marketing research. I understand that my/our/his/her first name and portrait, picture or video clips in all forms of media ("Likeness") are for internal use only and will not be used in any form whatsoever that is accessible to anyone other than the offices and employees of Get Pultz'D, LLC its consultants, investors, or potential investors, and its clients or potential clients, all of whom shall be under agreement not to disclose such use.

All other infomation as obtained by Get Pultz'D, LLC, or its affiliates, is understood to be confidential and is not permissible to be shared with any third party companies or indiviuals without my express permission.
